Herner Chiropractic is committed to protecting the privacy and security of our patients' personal and health information. This policy describes how we collect, use and safeguard that information in compliance with applicable law, including the Health Insurance Portability and Accountability Act (HIPAA).

Information we collect

We collect personal information such as your name, address, phone number, email address, date of birth and emergency contact details. We also collect health information, which is protected health information (PHI) under HIPAA: your medical history and treatment records, your diagnoses and treatment plans, and your insurance and billing information. This website itself collects none of this; see "This website" below.

How we use your information

We use this information to provide chiropractic treatment, to schedule and manage appointments, to send appointment reminders and treatment-related communications, to process billing and insurance claims, to improve our services, and to comply with legal and regulatory requirements.

HIPAA compliance

Herner Chiropractic complies with HIPAA to protect your PHI. We maintain administrative, physical and technical safeguards, limit access to PHI to authorized personnel, use secure systems including the ChiroTouch electronic health record platform, and do not disclose PHI without your authorization except as permitted by law. Our Notice of Privacy Practices, which describes these uses and your rights in detail, is available at the office.

Electronic communications

By providing your contact information, you consent to receive appointment reminders, scheduling confirmations and treatment-related notifications by email and text message. Text message frequency varies with your interactions with the office, and message and data rates may apply. You may opt out of text messages at any time by replying STOP, and you can get assistance by replying HELP or by contacting the office. While we take reasonable steps to secure communications, email and text messaging are not always fully secure, and by opting in you acknowledge and accept that risk. Sharing of information

We share your information only as necessary: with insurance providers for billing and claims, with other providers involved in your treatment, with service providers such as ChiroTouch that support our operations, and with legal authorities when required by law. We do not sell your personal or health information, and we do not share mobile phone numbers or text messaging consent with third parties for their marketing purposes.

Data security

We use industry-standard security measures, including encrypted systems and secure data storage, access controls and authentication, and regular monitoring and updates.

Your rights

You have the right to access your health records, to request corrections to your information, to request restrictions on certain uses or disclosures, and to request confidential communications. To exercise these rights, contact the office using the information below.

This website

This website has no contact forms, sets no cookies, and runs no analytics or visitor tracking. The "Get directions" link opens Google Maps only when you choose it; nothing from Google loads on this site on its own. Our web host keeps standard server logs, such as IP address and browser type, to operate and secure the site.

Changes to this policy

We may update this policy from time to time. Updates are posted on this website with a revised effective date.
